Your First Orthodontist Visit

Please assist us by providing the following information at the time of your first visit:

  • Any x-rays (if applicable)
  • A list of medications you are presently taking
  • If you have dental insurance, bring the necessary completed insurance cards and/or forms. This will save time and allow us to help you process any claims.
  • Completed printed patient medical history forms.
  • Your referral letter from your general dentist

 

Upon your arrival, you will be greeted by one of our treatment coordinators who will assist Dr. Freels during your first examination. Dr. Freels will perform a comprehensive assessment of your teeth and jaws, explain his initial findings, and suggest treatment options. Our treatment coordinator will discuss the available products, payment methods, insurance coverage, and the next steps with you. After addressing all your queries, our treatment coordinator will provide you with a printed estimate for your records. For patients keen on starting treatment promptly, diagnostic records can be gathered during the initial consultation, streamlining the process and saving time by consolidating two appointments into one.

Diagnostic Records & Treatment Planning

If it is determined that you are ready to start your orthodontic treatment, you will be scheduled for diagnostic records. These records consist of:

  • Intraoral photographs
  • Orthodontic x-rays
  • Study models
  • 3D scan option available
